What is buphthalmos?

Buphthalmos or "bull's eye" is an alteration that causes the growth of the eyeball as a consequence of congenital glaucoma. Congenital glaucoma is a disease that occurs due to a defect during the development of the embryo and can affect children between 0 and 3 years of age. 

Congenital glaucoma usually occurs when there is a defect in the drainage system of aqueous humor, the substance inside the eye. If the aqueous humor cannot flow, the intraocular pressure of the eye increases and causes very serious damage to the optic nerve and throughout the ocular structure. 

Among the complications that can arise as a consequence of congenital glaucoma, buthalmos stands out, which is the enlargement of the eyeball that causes the appearance of bulging eyes or out. 

Causes

In congenital glaucoma, unlike glaucoma in adults, elevated levels of intraocular pressure not only affect the optic nerve, but also cause lesions and changes in the entire ocular structure. 

One of the consequences of congenital glaucoma is distention of the eye walls, since these are more flexible during childhood because they are in the development stage. When the walls are distended they swell in such a way that it causes that illusion of an enlarged eye, or bull's eye. 

Diagnosis

The diagnosis of buftalmos can be made through a Clinical examination. In most cases, by looking at the child, the growth of the eyeball can be detected. 

In the presence of a buftalmos it is important examine the entire ocular structure completely to detect the health of the eyes and damage caused by glaucoma. Ideally, the ophthalmological examination should be performed by a pediatric ophthalmologist

Transmission

Fortunately, buphthalmos is a reversible condition that can be treated by operating on congenital glaucoma. The glaucoma surgery in children, the aim is to find an alternative to drainage of the aqueous humor and thus control the pressure levels of the eye. 

There are different techniques to operate on congenital glaucoma, but the most used is the goniotomy. This surgery consists of freeing the drainage angle of the eye of all the embryonic tissue that may be obstructing it. If the surgeon cannot access the drainage angle because the cornea is very inflamed, you can choose to perform a trabeculectomy

Trabeculectomy is one of the most used procedures to treat all types of glaucoma and consists of making an incision in the sclera to then form a filter bubble in the conjunctiva. through this filtration surgical technique we managed to filter the aqueous humor and monitor IOP levels.
