Dry eye treatment

Although the dry eye treatment It has evolved beyond artificial tears and anti-inflammatory and regenerative drugs, new technologies have appeared.

The most innovative is the treatment with High Intensity Pulsed Light (IPL), la Pulsed radiofrequency and Infrared laser. They are treatments that, despite the scientific evidence of their advantages, are still performed by very few centers. This is because expensive technologies and trained personnel are required. At IOD we have everything necessary to carry it out and in our case we have been pioneers at a national and international level.

These are treatments that are performed in consultation, such as thermal laser treatments in diabetics or YAG lasers. They are painless, do not present complications and lasts a few minutes.

From the interventionist point of view and where an operating room is required, new treatments stand out, such as Exfoliation of the free eyelid margin and Probing- Blepharoplasty of the Meibomian glands (glands that are arranged inside the eyelid). They are very innovative treatments with a high level of efficacy and safety. They are still carried out in very few centers but in the IOD we have the technology and personnel to carry them out.

Interventional treatment of the lacrimal ducts

La tear duct obstruction It is one of the most frequent pathologies in ophthalmology. In many cases it is not an irreversible obstruction, a fact that is confirmed by the radiological study with contrast and the CT scan to assess the tear sac and nasal turbinates. In cases of partial obstruction, probing treatment is performed and occasionally, it is accompanied by dilatation of the tear duct.

When the obstruction is complete, if it is located in the lacrimal canaliculi, it will be necessary to reconstruct these but, if the obstruction is located in the lacrimal duct, in the area of ​​the lacrimal duct close to the mouth of the nostrils (the most frequent), a classic reconstructive surgery can be performed or Dacryocystorhinostomy (DCR) or selective surgery using the Diode laser.

The DCR is very aggressive and has a high failure rate, whereas when it is performed endoscopically with the laser, it is much less aggressive, more efficient and with a faster recovery, as well as a much lower failure rate.

This type of surgery, more frequent in children, can also be performed in adults, even for aesthetic purposes. The additional fact of having an anesthesia team specialized in ophthalmology is also a differentiating factor, it reduces the anxiety of patients and especially parents.

Perhaps the most important advance in this campor is the combination of surgery, conventional or with botulinum toxin, with neurosensory therapies. We know that many patients with strabismus suffer from amblyopia (lazy eye), which hinders functional recovery and even induces the failure of surgery when a new deviation of the eyes becomes evident with the passage of time. Until that time, visual therapy exercises were carried out that helped in the treatment but were not very effective, so they were almost abandoned. Currently we have newer methods of exercises with computer screens that can stimulate, selectively, the different ways of visual perception, achieving results that, in isolation or, combined with surgery, greatly improve the results we achieved until the current moment.

At IOD we have the technologies and experts in the field to be able to carry out this type of therapy.

Treatments of vision and sport and vision and driving

We think that the IOD is the ideal framework to create these two units: vision and sport and vision and driving. They are transversal functional units, which involve different departments of the ophthalmology service and even of other Dexeus services (endocrine, psychology).

We have seen that every time there is more demand for people who want to improve their visual levels when doing sports, from elite athletes to young people, who want to perform refractive surgery and know that dry eyes or other problems may appear, not serious but that make it difficult practice sports such as tennis, skiing, etc. In the same way, there are people who after cataract or other surgery have changes in visual perception and are not comfortable with their vision in sports or do not feel safe driving (reflections with lights, calculation of distances, etc).

All these cases require a specific study, with a technology and experts in this field, to establish a treatment guideline that improves the conditions of visual perception in each person.

The treatments include from the drugs to improve the tear, the pupilar dynamics, therapies of neurosensory exercises to improve the reflections and the sensitivity to the contrast, until the specific surgical treatments. In the cases of surgery, it is about offering treatments such as refractive surgery, but with less aggressive techniques and offering better visual quality.

Treatment of childhood myopia

The treatments of myopia in children they deserve a separate mention because it is something new and is a great demand for patients. They are combined treatments with drugs (eye drops) and sensorineural stimulation exercises, which slow down the evolution of myopia.

In many cases it is combined with the adaptation of lenses that control the accommodation of the eyes or contact lenses that are used only at night to mold the cornea and correct myopia, which is what is called Orthokeratology.
